Immunomodulation During Systemic Inflammation

Peroxisome proliferator-activated receptor-γ (PPARγ) regulates the sepsis-induced inflammatory response. The objective is to determine if PPARγ is affected in sepsis. The hypothesis is that PPARγ is diminished in blood cells of septic children. In a pilot study, cases were patients with sepsis and controls were without signs of inflammation. Cecal ligation and puncture (CLP) caused sepsis in rats which were sacrificed at various time points. Cells, layered onto Percoll for monocyte isolation, were fractionated and plasma obtained for cytokine analysis. PPARγ was evaluated by western and electrophoretic mobility shift assay. CLP decreased PPARγ and 15-deoxy-Δ&sup12,14;-PGJ2. Cytokines were elevated in septic shock and decreased in the resolution of sepsis. Total and high molecular adiponectin were higher in the resolution of sepsis. The PPARγ pathway is affected in sepsis. These results can lead to novel therapies for sepsis, including the use of FDA-approved PPARγ agonists, thiazolidinediones, which can augment the PPARγ pathway.
