Comparative study of the topical treatments for human papilloma virus infection of the vulva

Introduction: Infection by papilloma virus, which affects different areas of the lower genital tract, has great prevalence in the female population in fertile age, in which it has been related to the developement of malignant and premalignant processes. The objective of this study is to know the therapeutic and adverse effects derived from the topic use of 5-fluorouracil and imiquimod in the treatment of both clinical and subclinical vulvar papillomas. Method: A quasi-experimental 3 year study was carried out with 330 patients (198 treated with imiquimod and 132 with 5-fluorouracil) who had been diagnosed vulvoscopically as suffering lesions caused by papillomavirus, and whose follow-up included an interview and a vulvoscopic exploration every two weeks, or in the case of any adverse effects (pruritus, burning sensation, pain or stinging sensation). Results: In both groups it was seen that the best response to the treatment was during the first 4 weeks, with no significant differences observed between them. Among the adverse effects the group treated with imiquimod presented pruritus as the most common symptom (52.02%) associated with a burning sensation (46.46%) and stinging sensation (44.44%), while in the patients treated with fluorouracil stinging (46.21) and burning sensation (44.69%) were the predominant effects as well as moderate pain. Conclusions: Both 5-fluorouracil and imiquimod are equally effective in the treatment of vulvar lesions caused by the human papilloma virus, the safety profile of both of them being similar, even though the local effects in the application area are usually more intense in the case of 5-fluorouracil.
