We occasionally encounter difficult cases of stent and balloon delivery to the distal lesion due to severe calcification or tortuosity of the proximal section. We describe a novel mother and child technique with a 4F inner catheter based on proper alignment of both catheters to deliver balloon, stent, and guide catheter to the distal lesion. © 2011 Wiley-Liss, Inc.
