BCR/ABL fusion gene was detected in acute promyelocytic leukemia (APL): a case study on clinic and laboratory results.

The prognostic risk of acute myeloid leukemia (AML) is related with various chromosome abnormalities. In adult population, the presence of t (9; 22) (q34; q11) predicts a poor prognosis while t (15; 17) (q22; q21) usually indicates a good prognosis. Concurrence of both abnormalities is rare, with unknown prognosis. We report a case of AML in a 48-year-old male. Bone marrow cytology showed 91.5% promyelocytes, and POX-positive. The cytogenetic result is: 46, XY, t (9; 22) (q34; q11), t (15; 17) (q22; q21) [10] / 47, idem, +8 [4] / 46, idem, der (14) t (9; 14) (q10; q10) [6]. Flow cytometric analysis revealed a large population (90.5% of total events) of leukemia cells with the immunophenotype of CD13+, CD33+, CD38+, CD64+, MPO+, CD4+ (partial), CD11b+ (partial), CD117+ (small subset), CD34- and HLA-DR-. FISH and Real-time PCR analysis demonstrated that the bone marrow cells were positive for BCR/ABL and PML/RARA fusion genes. The patient was diagnosed as acute promyelocytic leukemia (APL), and treated with all-trans retinoic acid (ATRA) in combination with arsenic trioxide, as well as oral administration of imatinib. After two-month treatment, the patient achieved complete remission (CR). There is no evidence of recurrence after 18 months.
