Is the discussion of patient cases in clin-ical ethics-committees useful?

Background. All Norwegian hospital trusts currently have at least one clinical ethics committee (CEC). Part of their mandate is to advise and guide hospital clinicians in ethically challenging situations. We wanted to find out if clinicians who had used CECs in situations regarding individual patients found it useful and worthwhile. Material and methods. The local CECs were requested to forward a questionnaire to all clinicians who had discussed an individual patient case in a CEC during the foregoing 18 months. The survey was anonymous. 43 of 86 questionnaires (50 %) were completed and sent to the Centre for Medical Ethics at the University of Oslo. Results. Most clinicians contacted CEC for several reasons, the wish to have a broad discussion of the case was the most common reason. They found the deliberation to be useful in relation to this. Other ethical challenges that were often discussed were limitation of medical treatment for seriously ill patients (56 %), next-of-kin's will (40 %) and patient autonomy (37 %). CEC gave advice in 50 % of the cases, and 38 % of the deliberations had practical consequences, for example discontinuation of medical treatment (in six cases). Interpretation. The results should be interpreted in light of the low response rate. The CEC work is mostly evaluated as useful, and the deliberations may have practical consequences. It is a challenge to make clinicians aware of the work in CECs and to quality assure the work.
