Abstract This case report describes a woman at 35 weeks gestation, who presented with an acute type A aortic dissection involving the total aortic arch and descending aorta. She underwent a successful ascending aorta replacement, total arch replacement, and stented elephant trunk implantation at the time of cesarean section with favorable maternal and fetal outcomes.
