Spontaneous Hemorrhage of a Parathyroid Adenoma into the Mediastinum.

Objectives: We describe the case of a previously healthy 56-year-old female who presented with acute, non-traumatic, left neck pain and dysphagia.Methods: We report the results of the physical examination, imaging studies, and clinical laboratory studies, and we also describe the patient's hospital course.Results: We report a patient who sought medical attention due to acute, non-traumatic neck pain and dysphagia and was subsequently found to have acute extracapsular hemorrhage of a parathyroid adenoma. Imaging studies revealed a mass effect starting in the neck and extending into the mediastinum. Surgical neck exploration and histolopathologic evaluation confirmed the diagnosis of spontaneous rupture of a parathyroid adenoma.Conclusion: Extracapsular hemorrhage of a parathyroid adenoma is rare and requires a high index of clinical suspicion for diagnosis. Diagnosis should be considered in a patient who presents with acute anemia, hypercalcemia, and neck mass.
