Sialendoscopy: a minimally invasive treatment for salivary gland obstruction.

Background:  There are approximately 1200 patients newly diagnosed with sialolithiasis per year in the UK. Sialoadenectomy is considered the definitive treatment option for sialolithiasis, however it is associated with low but significant morbidity. Objective of review:  To introduce sialendoscopy as an alternative for both diagnosis and treatment of salivary gland obstruction. Type of review:  Systematic Review Search strategy:  An up to date literature search using Medline and EMBASE Evaluation Method:  Only studies published in the English literature and retrieved in full were analysed. Studies where results are combined with intra-oral stone excision were excluded. Results:  Eight of the largest case series with 60 or more adult cases and two further paediatric case series are summarised. The efficacy of sialendoscopy for diagnosis of salivary gland obstruction was high ranging from 85-100%. The success in intervention varied with the cause of obstruction but overall ranged from 62-93%, with some patients requiring more than one operation to achieve success. A framework for surgeons considering adopting sialendoscopy is provided by discussing; the technique, appropriate patient selection, benefits and drawbacks when compared to standard methods of treatment, setup costs and training required. Conclusions:  Sialendoscopy is a gland preserving, day case, surgical technique with low morbidity and proven efficacy for the first line treatment of major salivary gland duct obstruction. © 2012 Blackwell Publishing Ltd.
