Sex difference in lipid profile in relation to the progression of glucose abnormalities.

Objectives:  Since type 2 diabetes mellitus (DM2) abolishes the lower cardiovascular risk of females, we investigated the role played by changes in blood lipids. Methods:  In 1091 consecutive subjects, 478 men and 613 women, we executed OGTT to subdivide them in: normal glucose tolerance (NGT, n=589); pre-diabetes (pre-DM2) including impaired fasting glucose (IFG, n=212), impaired glucose tolerance (IGT, n=84) and both (IFG/IGT, n=102); DM2 (n=104). In each subject we measured: total cholesterol, triglycerides, HDL-cholesterol, LDL-cholesterol, apolipoproteinA-I, apolipoproteinB, apoB/apoA-I ratio. We tested by Student's t test the statistical differences in lipids between men and women in each of the aforementioned groups. Results:  NGT male subjects presented higher triglycerides and apoB/apoA-I ratio (p<0.0001), lower HDL-cholesterol and apoA-I (p<0.0001). The pre-DM2 men maintained a higher apoB/apoA-I ratio (p<0.05) and lower HDL-cholesterol (p<0.0001). In DM2 group only HDL-c was lower in men (p<0.05). Conclusions:  The progression of glucose intolerance, from NGT to pre-DM2 and DM2, exhibits striking sex difference regarding the lipid profile. Our data demonstrate a worsening of plasma lipid composition in women who become diabetic. This could explain, at least in part, the loss of the more favourable cardiovascular risk present in NGT females.
