Risk factors and diabetic retinopathy.

Dear Editor, We thank Dizdarevic et al. for the article investigating the systemic risk factors of diabetic retinopathy (1). Methodology of the study is simple and easy to perform, however it carries an important epidemiologic result. Similar to previous studies (2,3) the authors found that disease duration was the most important factor. The authors also found that systemic arterial blood pressure and HbA1C were also associated with increased risk. However, there was no relation between diabetic retinopathy and serum cholesterol and triglyceride levels. These indings are closely correlated to current knowledge. In chronic diseases such as Diabetes Mellitus, some clinical conditions and/or laboratory indings may be a result of some other primary changes. A study reported that, in addition to secondary vascular lesions in diabetes, some primary retinal neurodegeneration begins in the very early stages of the disease (4). Why is that so important? From a therapeutic point of view, the clinician should be alerted toward the primary changes (5). For this reason, we would kindly ask the authors to perform the correlations between the disease duration, systemic arterial hypertension and serum lipid levels. These indings will not only cause scientiic contribution to the paper, but also be important in diabetic retinopathy cases.
