Relationship between serum asymmetric dimethyl-arginine and left ventricular structure and function in patients with end-stage renal disease treated with hemodialysis.

INTRODUCTION: Asymmetric dimethylarginin (ADMA) is an endogenous inhibitor of nitric oxide synthase, considered an effector of endothelial dysfunction.Among multiple diseases associated with elevated ADMA, chronic renal disease is often quoted, and ADMA is thought to be related to certain adverse effects of chronic uremia. Association between left ventricle (LV) structure and function and ADMA has been studied in many diseases,but only few of them addressed this issue in ESRD. OBJECTIVES: Objective of the study was to analyze associations between ADMA, LV geometry and function in patients with ESRD. PATIENTS AND METHODS: Study group consisted of 56 patients aged 59.0±13.1 years on HD for 70±67 months. Serum ADMA and multiple biochemical parameters were assayed and echocardiography was performed. RESULTS: The mean sADMA in patients equaled 2.39±1.0 μmol/ and was significantly higher as compared to control group (0.55±0.12 μmol/l; p< 0.01). Based on echocardiography we categorized our patients into the following groups: normal (17.8%), concentric remodelling (8.9%), concentric hypertrophy (35.7%), excentric hypertrophy (37.5%), impaired systolic function (10.7%), and impaired diastolic function (71.4%) (one could fit one or more groups). sADMA correlated with mean (r=0.78; p<0.05) and relative LV wall thickness (r=0.64; p<0.05) and with LVMI (r=0.65; p<0.05), but not with indexes of systolic and diastolic function.sADMA was significantly higher in patients with excentric hypertrophy, concentric remodeling and concentric hypertrophy, being highest in the latter group, as compared to those with normal LV. sADMA was also shown to be independently associated with LVMI and relative and mean LV wall thickness.  CONCLUSIONS: Our study demonstrated association between sADMA and LV disturbances in patients with ESRD on HD.
