I read with interest the recent review by Marzuillo et al (1), but felt that there were some issues that needed to be clarified. The authors stated that there were only three case reports in the literature of therapeutic doses of tramadol inducing respiratory depression in adults. In our study of 525 patients with pure tramadol ingestion, 19 (3.7%) experienced apnoea, including three adolescents, and two of the patients used tramadol within therapeutic doses (2).
