To the Editor: Parent et al. (July 7 issue)(1) report that echocardiographic evaluation alone had a low positive predictive value for the detection of pulmonary hypertension. An alternative explanation is that the use of only one measurement from the complete echocardiographic evaluation may provide misleading estimates of pulmonary arterial pressure. Using only the tricuspid-valve regurgitant jet velocity, the authors may have missed persons with elevated pulmonary arterial pressure.
