A Patient With a Preexisting (Nontoxic, Nonautonomous) Thyroid Nodule and Concurrent Thyroiditis Mimicking a Toxic Solitary Autonomous Thyroid Nodule on 99mTcO4 Scintigraphy.

A 40-year-old patient had been analyzed because of a solitary nodule in the thyroid, including thyroid scintigraphy with TcO4 showing only slightly decreased uptake in the lower right lobe in an otherwise normal thyroid. Five years later, this patient presented with hyperthyroidism and a painful neck. This time thyroid scintigraphy showed single focal uptake in the lower right lobe and suppression of the remaining thyroid tissue, suggesting a toxic solitary autonomous nodule. Clinically, however, the patient was suffering from viral thyroiditis, and she recovered spontaneously. A specific pitfall of thyroid scintigraphy is discussed.
