To the Editor: In their study of oral acyclovir in neonates after herpes simplex virus (HSV) infection, Kimberlin et al. (Oct. 6 issue)(1) describe an initial dose of parenteral acyclovir (300 mg per square meter of body-surface area) that is lower than that used in current clinical practice (1500 mg per square meter) administered for 21 days in cases of central nervous system involvement.(2) In addition, the authors do not mention the possible genetic predisposition to HSV among infants with frequent skin relapses(3) and imply that 6 months of oral prophylaxis may be adequate.
