The digastric corset is a new concept for open neck contouring, indicated in the treatment of difficult necks. We described a retaining ligament between the anterior belly of the digastric muscle and the mylo-hyoid muscle. After suprahyoid fat lipectomy, a digastric corset can be performed as a running suture between the two retaining ligaments, which tighten the floor of the mouth, performed through a small incision behind the retro-chin sulcus. The digastric corset anchored on the mylohyoid muscle associated to lateral platysma-suspension gives a well-defined anterior cervical angle, sign of a youthful-looking neck. This article describes the surgical technique and discusses the indications.
