Normal saline bolus infusion for hypoperfusion in the newborn.

Normal sa line (NS) bolus infusions are indicated during resuscitation of the newborn to treat hypovolemia and resulting lactic acidosis because of inadequate tissue perfusion. However, in the neonatal intensive care settings, NS bolus infusions are also used to treat clinical hypoperfusion in the absence of confirmed volume loss or confirmed hypovolemic shock. A small but significant number of term and late preterm newborns exhibit severe pallor after birth secondary to hypoperfusion and/or compensated shock. Although many of these infants have no apparent clinical evidence of overt volume loss, volume expansion with NS is a common treatment to reverse the progression to uncompensated shock supported primarily by empiric clinical evidence and resuscitation guidelines. This column will review the available evidence and examine the theoretical clinical indications for NS bolus infusions as well as suggest measurable clinical signs of response in the term and late preterm newborns.
