OBJECTIVE: To assess cartilage GAG content and cartilage thickness of the metacarpophalangeal (MCP) joints in patients with early rheumatoid arthritis (ERA) compared to healthy volunteers. METHODS: After review board approval and informed consent, 22 subjects were prospectively enrolled (9 ERA patients: 7 female, 2 male; 49 ±13 years, range 25-68; 13 healthy volunteers: 10 females, 3 males; 51 ±12 years, range 25-66). In 44 joints measurements of cartilage thickness and dGEMRIC index (T1 [ms]) with the variable flip angle method of MCP joints of the index and middle finger were performed on a 3 Tesla MR scanner. MRI was evaluated for bone edema, erosions and synovitis (RAMRIS criteria). Student's T test was used to test for differences between groups. RESULTS: Mean dGEMRIC index was 497 ± 86 ms in healthy volunteers and was significantly lower in the ERA group (417 ± 80 ms) (p= 0.046). There was no joint space narrowing on standard radiographs. No significant difference was found between cartilage thickness of ERA patients (index finger: 1.27 ± 0.23 mm; middle finger: 1.26 ± 0.23 mm) and healthy volunteers (index finger: 1.46 ± 0.34 mm; middle finger: 0.97 ± 0.47 mm) (p= 0.16; p= 0.10). No significant correlation was noted between cartilage thickness and dGEMRIC index (R= 0.36, p= 0.88). CONCLUSION: In this pilot study, cartilage damage was present in MCP joints of ERA patients despite the absence of joint space narrowing on standard radiographs and MRI. Cartilage damage in RA can be imaged with dGEMRIC. |
