Magnetic resonance imaging of the normal fetal oesophagus.

Abstract/OtherAbstract:
PURPOSE: To establish the magnetic resonance imaging (MRI) appearance of the normal fetal oesophagus and to supply metric data on its growth. MATERIAL AND METHODS: In 250 fetal MRI studies (20-39 gestational weeks) the visibility of the upper (O1: cervical to tracheal bifurcation) and lower oesophageal segment (O2: tracheal bifurcation to diaphragm) was assessed separately in three section planes and scored as being visible, whether fluid-filled or not. The maximum diameters were recorded and dynamic studies were analysed for the duration of fluid-filling. RESULTS: O1 was only visualized when fluid-filled and was visible in 16.8% (axial), 4% (frontal) and 22.5% (sagittal). O2 could also be detected in the solid state, and was seen in 96.4% (axial), 74% (frontal) and 39.6% (sagittal) of cases, being fluid-filled in 62.7-88.9%. Depending on the amount of fluid-filling O2 had a variable appearance. On dynamic scans O1 was seen fluid-filled for 0.5-1.5 seconds, O2 for 3-35 seconds. The frequency of visualization increased with gestational age, as did the diameters, showing a linear increase. CONCLUSION: Visualization of the fetal oesophagus with MRI depends on section plane, segment and gestational age, with significant differences between the two parts: While O1 requires fluid-filling related to fetal swallowing, the O2 portion usually can also be demonstrated independent of fluid-filling as a consequence of its topographical relationships. The frequent distended condition of O2 may reflect the immature nature of oesophageal neuromuscular activity, and the lack of an anti-reflux mechanism due to a short abdominal oesophagus.
