Long term functional and quality of life evaluation after treatment of advanced pharyngolaryngeal carcinoma.

OBJECTIVE: To compare total laryngectomy (TL) vs. concurrent chemoradiation (CCR) for function and long term quality of life (QoL) outcomes in pharyngolaryngeal carcinoma patients.
METHODS: Among 145 patients treated for advanced pharyngolaryngeal cancer by CCR or TL between 2000 and 2008, 47 disease free patients, who had completed treatment for more than a year, were evaluated for function and QoL using the EORTC QLQ.
RESULTS: No significant difference was observed between TL group (26 patients) and CCR group (21 patients) concerning feeding-tube (p=0.72), oral supplements (p=0.84) and pneumopathies (p=1.00). Laryngeal functional rate after CCR was 72% at 2 years. No significant difference was observed between the two groups when using EORTC QLQ C-30 scales (p≥0.05). According to EORTC-H&N35 questionnaire, surgical patients reported significantly greater difficulties with smell and taste (p=0.005) and chemoradiation patients with dry mouth (p=0.010) and weight loss (p=0.022). Differences that seemed clinically meaningful but not statistically significant emerged on several other scales.
CONCLUSIONS: Both TL and CCR led to a similar high rate of dysphagia-related morbidity (feeding tube, oral supplements, and pneumopathies) and QoL alteration but the groups differed in the specific QoL domains affected.
