Left main minimal plaque burden complicated by an acute massive thrombosis: diagnostic and therapeutic strategies.

Acute coronary syndrome because of left main coronary artery (LMCA) occlusion is a catastrophic event associated with poor prognosis and high in-hospital mortality. The most common cause for LMCA thrombosis is an atherosclerotic plaque rupture with subsequent superimposed thrombus formation. If this cause is excluded, rare causes should be investigated (hematologic diseases, tumors and embolism). If these clinical conditions are excluded, a trivial plaque burden complicated by thrombosis might be supposed. We report here a rare case of an acute coronary syndrome as a result of a massive thrombosis of the left main coronary artery without any significant plaque burden detectable by angiography. The appropriated diagnostic and therapeutic tools are discussed.
