Glucagon-induced Pheochromocytoma Crisis: Case Report and Literature Review.

Objective: To report a previously asymptomatic woman, who developed glucagon-induced pheochromocytoma crisis during preparation for elective colonoscopy.Methods: The clinical features, laboratory and imaging findings as well as outcome are presented and the related literature is reviewed.Results: A 76-year-old woman received glucagon prior to routine colonoscopy to inhibit intestinal motility. She immediately developed severe hypertension, cardiac arrhythmias and altered mental status. Her hospital course was complicated by encephalopathy, cardiac, respiratory, renal and hepatic failure. A CT scan of abdomen revealed a 6.5 × 4.8 cm mass in the left adrenal gland. Lab results for vanillyl mandelic acid (VMA), catecholamines and metanephrines were markedly elevated. She underwent a successful laparoscopic left adrenalectomy. Histopathology of the adrenal mass was diagnostic of a pheochromocytoma.Conclusion: Glucagon administration activated an occult pheochromocytoma causing multi-organ injury due to a catecholamine crisis. It is imperative that health care providers using glucagon be conscious of this rare but life-threatening complication.
