Getting paid for taking ER calls: who decides, and how much is fair?

Most physicians have voluntarily taken calls at hospital emergency rooms because of medical staff bylaw requirements, a sense of professional or ethical obligation or civic commitment. Today, however, most specialists expect to receive some form of remuneration for taking ER calls. Physicians' desire for such a stipend obviously must coincide with a hospital's willingness to pay--and this leads to the key question of the fair market value, or reasonableness, of a specific compensation level.
