Pediatric laboratory medicine is a discipline which encompasses monitoring the normal growth and development of the ultimate dynamic system, the human body, as well as detecting and diagnosing aberrations in that system. Basic metabolic processes and body chemistries change and adapt to accommodate growth as a newborn infant develops through childhood and becomes an adult. At many points along this developmental continuum the metabolic responses and capabilities are significantly different from those of an adult. Thus the pediatric laboratory is unique in that it provides testing for human beings in which nothing is static. The complexity of growth and differentiation of the newborn, child, and teenager means that the laboratory must have processes to accommodate collecting, running, and reporting samples for patient who are vastly different from year to year, or even day to day in the case of the newborn. This chapter will provide information to the practicing pathologist to assist in understanding issues which occur in a typically high volume, automated Core Laboratory which performs testing for routine chemistry and hematology.
