Ethnic Disparity in Hemoglobin A1c Levels Among Normoglycemic Offspring of Parents with Type 2 Diabetes.

Objective: Black persons have higher hemoglobin A1c (HbA1c) values than white persons with similar blood glucose levels, but these data have been derived from populations with variable glycemic status and largely unknown familial history of diabetes. We therefore investigated racial disparities in HbA1c among nondiabetic persons with similar parental history of type 2 diabetes (T2DM).Methods: We studied a community-based sample of 302 (135 white, 167 black) adult offspring of parents with T2DM. Measurements included anthropometry, hematology, serial fasting plasma glucose (FPG), oral glucose tolerance test (OGTT), plasma insulin, HbA1c, and insulin sensitivity and β-cell function, using homeostasis model assessment (HOMA).Results: Compared to whites, black offspring had lower FPG 91.9 ± 0.51 mg/dl vs. 93.6 ± 0.50 mg/dl, P=0.015), lower area-under-the-curve (AUC) of plasma glucose during OGTT (P=00019), higher body mass index (BMI) (31.1 ± 0.61 vs. 28.5 ± 0.57 kg/m2, P=0.0006), and similar insulin sensitivity and β-cell function. HbA1c was higher in blacks than whites (5.68 ± 0.033% vs. 5.45 ± 0.028%, P <0.0001). The absolute black-white difference in HbA1c level of ~0.22% persisted after adjusting for age, hemoglobin, hematocrit, BMI, girth, FPG, glucose AUC, and other covariates.Conclusion: Among healthy offspring of parents with T2DM, African Americans have higher HbA1c levels than white persons, after adjusting for age, adiposity, blood glucose and known variables. Thus, plasma glucose level would be more valid than HbA1c for diagnosing prediabetes or diabetes in black persons.
