To the Editor: Afdhal et al. (Aug. 23 issue)(1) used eltrombopag to increase the platelet count and reduce the need for transfusion in patients with cirrhosis who were undergoing elective invasive procedures. The hemostatic system of patients with cirrhosis is in a "rebalanced" state, owing to concomitant alterations in prohemostatic and antihemostatic pathways.(2),(3) This raises questions regarding the need to increase the platelet count in patients with cirrhosis before invasive procedures.(3) Bleeding events in the eltrombopag and placebo groups were similar, despite a substantial difference between the preprocedural platelet counts in the patients in the two groups; this result . . .
