To the Editor: Kang et al. (June 28 issue)(1) report significantly better outcomes with early surgery than with conventional treatment in patients with infective endocarditis and vegetations with a diameter greater than 10 mm. More than 90% of the patients in their study had severe aortic-valve or mitral-valve regurgitation; however, it remains unclear whether these patients had a concomitant class I indication for surgery.(2) Specifically, data such as left atrial or pulmonary pressures, which are often elevated with severe aortic or mitral regurgitation, were not provided.(3) Furthermore, severe valve regurgitation, even in the absence of heart failure, constitutes a class . . .
