Compensatory enlargement of a solitary functioning kidney during fetal development.

OBJECTIVE: To determine the time of onset and degree of compensatory enlargement of the fetal solitary kidney in fetuses with unilateral renal agenesis (URA) or unilateral multicystic kidney dysplasia (MCKD). METHODS: Of 67 fetuses, 60 MCDK and 7 URA and no other congenital anomalies, a total of 147 kidney length measurements were obtained retrospectively in our prenatal ultrasound database. A nomogram was established and compared to nomograms of normal kidney length. RESULTS: Compensatory enlargement, defined as renal length >95(th) percentile for gestational age was demonstrated in 87% and 100% of the MCKD and URA cases respectively, leading to a total of 88%. In the 38(th) week of gestation the mean enlargement, defined as mean renal length of a SFK >50(th) percentile for normal renal length, was 22.2% (9.61 mm) and the mean compensatory enlargement, defined as mean renal length of a SFK >95(th) percentile for normal renal length, was 11.3% (5.25 mm). Compensatory growth of solitary kidneys is demonstrated from the 20(th) week of gestation and onward. CONCLUSION: This study demonstrates compensatory enlargement of a SFK from the 20(th) week of gestation and onward, occurs in 88% of the fetuses and has a mean enlargement of 11.3% by the 37(th) week of gestation.
