Editor's Note: The following is the commentary of a young psychiatrist about the evolution of his thinking during training. He offers a provocative and fresh view of a complicated illness which affects more and more of us as we grow older. In a psychiatry residency outpatient clinic, an elderly woman with dementia and depression is successively treated with medication management, cognitive behavioral therapy, and psychodynamic psychotherapy. In the course of the treatment, the patient and her therapist struggle with what she can hope for as her dementia progresses. The use of a transference-based psychotherapy deepens and complicates the interactions between patient and therapist, challenging the hopes of patient and therapist alike. |
