A Case Report of Acute Necrotizing Pancreatitis Associated with a Combination Treatment of Sitagliptin and Exenatide.

Objective: To report the first post-marketing case of necrotizing pancreatitis in a patient on a combination of sitagliptin and exenatide.Methods: We describe the patient's clinical presentation, laboratory test results, imaging and autopsy findings of the study patient.Results: A 76-year-old woman with a history of type 2 diabetes mellitus presented with severe abdominal pain, vomiting, and fever requiring hospital admission. She had been treated for 3 years with exenatide for management of her diabetes mellitus. A few weeks before presentation, sitagliptin was added, presumably to further optimize her glycemic control. She was diagnosed with acute pancreatitis during the admission. At initial presentation she had a serum amylase of 1136 U/L (normal 10-130) and a lipase of over 3500 U/L (normal 0-75). In addition, computed tomography (CT) scan of the abdomen and pelvis demonstrated extensive pancreatic parenchymal necrosis. After initial brief improvement, her symptoms worsened and despite aggressive care, her clinical state deteriorated and she expired. Autopsy findings demonstrated acute necrotizing pancreatitis with complete digestion of the pancreas.Conclusions: The patient described in this case had no identifiable cause for her pancreatitis except a history of diabetes mellitus. She had undergone prior cholecystectomy, denied alcohol consumption, and had a normal lipid profile. Considering the temporal relation of the symptoms to the onset of therapy, this case strongly suggests a possible causal link between exenatide or sitagliptin or the combination of the two as the etiology of pancreatitis in this patient.
