A Case of Primary Leptomeningeal Melanoma Evaluated on FDG PET/CT.

Primary leptomeningeal melanoma is a rare and aggressive tumor caused by melanin containing cells in the pia mater, which may malignantly degenerate. Despite the advances in neuroimaging, it remains a difficult diagnosis. We report a case of 51-year-old man who presented with headache, nausea and vomiting, fatigue, and left hip pain. MRI showed diffuse disease of the leptomeninges, most notably in the cauda equina. Subsequent PET/CT was performed, in part to identify a site of suitable biopsy. No disease was identified outside the central nervous system, and open biopsy of the cauda equina demonstrated primary leptomeningeal melanoma.
