To the Editor: Olesen et al. (Aug. 16 issue)(1) report that in their study, warfarin treatment was associated with a decreased risk of thromboembolic events among patients requiring renal-replacement therapy. In contrast, three large registry studies concluded that in patients with atrial fibrillation who were undergoing hemodialysis, warfarin was associated with an increased risk of stroke.(2)-(4) In addition, generally in patients who are receiving hemodialysis and warfarin, the risks of bleeding, cardiovascular calcifications, and calciphylaxis markedly increase. We therefore have argued against the use of warfarin in patients with atrial fibrillation who are receiving hemodialysis.(5) There are important limitations . . . |
