Artery of Adamkiewicz-sparing endovascular stent placement.

We report a case of successful endovascular treatment of an acute, uncomplicated Stanford type B aortic dissection, sparing the artery of Adamkiewicz. The ECG-gated MSCT diagnosed the reason for the acute chest pain in the patient, and identified the intercostal artery from which the artery of Adamkiewicz originated, thus permitting the positioning of the stent-graft well away from it. Ten months after successful stent-graft implantation the patient was well, the haematoma in the formerly dissected aortic wall had been almost completely re-absorbed and the false lumen had disappeared. A kink and fold of the stent-graft due to the concave path of the distal aortic arch remained stable and free of thrombi.
