Anticoagulant Therapy for Venous Thromboembolism during Pregnancy - Implications for Neuraxial Analgesia/Anesthesia.

We read Romualdi et al's recent systematic review and meta-analysis of anticoagulant therapy for venous thromboembolism (VTE) during pregnancy (1) with great interest. They conclude that anticoagulant therapy (primarily low molecular weight heparin [LMWH] in > 80% of cases) is safe and effective with a low incidence of antenatal (1.41% weighted mean incidence [WMI]) and early postnatal (1.9% WMI) major bleeding. While this data is reassuring, as anesthesiologists we are both curious and concerned about the implications for safe provision of neuraxial (primarily epidural) labor analgesia and/or anesthesia for parturients receiving LMWH. © 2013 International Society on Thrombosis and Haemostasis.
