Pulmonary sarcoidosis is a granulomatous disease of the lung parenchyma, which usually presents in a chronic, insidious fashion. Rarely though it can present as acute hypoxemic respiratory failure. We describe a case of a 40-year old man, who presented with acute respiratory failure that was ultimately proven to be sarcoidosis on a surgical lung biopsy. He showed dramatic response to corticosteroids. The possible hypotheses leading to acute respiratory failure in sarcoidosis are discussed along with a systematic review of the literature. |
